Instructions to complete the FJSNA Membership Application:

1. Select COUNTRY/ORGANIZATION (drop down menu)
2. For renewal from March 1st 2008 to February 28th 2009: nothing to select
3. For different timeframe: Select BEGIN and END of membership (drop down menu)
4. Select AMOUNT TO PAY (drop down menu) – 5.00 per month
5. Fill in NAME and RANK/PAY GRADE
6. Select BRANCH OF SERVICE (drop down menu)
7. Fill in date FIRST JOINED (mm/dd/yy) if not known leave blank
8. Fill in END of TOUR (mm/dd/yy) if not known leave blank
9. Fill in HOME ADDRESS
10. HOME TELEPHONE number without spaces: for example 3016106417, (column is self formatting)
11. SPOUSE NAME, CHILDREN NAMES, BIRTH YEAR, GENDER (drop down)
11. OFFICE ADDRESS
12. OFFICE PHONE and FAX: no spaces (self formatting)
13. PRINT, SIGN and ATTACH PAYMENT (payable to: “FJSNA”)
14. HAND IT OVER TO YOUR COUNTRY/SERVICE REP
15. KEEP A COPIE FOR YOUR RECORDS
